About the Position:
The Housing Navigation and LifeTrust Care (LTC) Specialist provide coordination and support to enable clients/family transition into their own homes in a community of their choice and provide case management services. The position would overlap with and provide Independent Living Service (ILS) support as needed.
Education:
- Bachelor's degree (exceptions can be made by the Executive Director).
Experience:
- Three years of experience working in human services; experience working with programs such as IHSS and Medi-Cal preferred.
- Experience with navigating affordable housing and working with unhoused populations.
Schedule:
- Full-Time schedule.
- Monday-Friday, between the hours of 9:00AM-7:00PM (flex).
- The work phone is on by 8:30AM.
Locations:
- Requires travel throughout Alameda County, including: Oakland, Berkeley, Emeryville, San Leandro, Castro Valley, Hayward, Dublin, Pleasanton, Livermore, and Fremont.
- Possession of a valid California driver's license, a driving record that meets agency standards, auto insurance, and access to a vehicle.
Compensation: $28.92 per hour.
Duties and Responsibilities:
- Provides individualized instruction to consumers with respect and sensitivity to each consumer’s needs and preferences.
- Housing/LTC Specialist hours of support will be assessed based on the needs and desires of the individuals served. The hours of employment, as well as job responsibilities, will be determined based on those needs and desires.
- Develop and maintain structure, implementation, and routine of client support.
- Identify unmet client needs and report back to the Director and Assistant Director.
- Responsible for communicating all information relevant to supporting the consumer to appropriate agency staff in a timely manner.
- Maintains a consistently positive attitude, which is imperative to successfully supporting each consumer.
- Responsible for scheduling direct service time to accommodate consumer needs.
- Provides vacation/sick coverage for other ILS staff as assigned.
- Completes EBI Training curriculum and other required training. Completes EBI shadowing as set forth by the supervisor.
- Knowledge of community resources.
- Participates in staff meetings as assigned.
- Identifies appropriate permanent housing options for clients such as subsidized housing, Section 8, BMR, 811 subsidy, and Project-Based units. As well as permanent supportive housing, affordable and market-rate housing, and other housing opportunities.
- Become active in the landlord community to identify housing opportunities for households experiencing homelessness and those at risk of becoming homeless with barriers.
- Provides information, referrals, linkages, and advocacy to assist clients in accessing services and resources. i.e., SSA, CalFresh, Medi-Cal, IHSS, affordable housing, and Community Health Services.
- Serves as the primary point of contact for landlord's concerns and complaints, acting as a liaison to connect landlords, and tenants.
- Exceptional Communication and organizational skills. Maintains accurate program-required documentation, case notes, reports, billing information, expenses, and updated daily calendar.
- Assists clients with procuring necessary documents and services such as identification cards, birth certificates, social security, and disability income.
- Provide coordination and support to enable clients to transition into affordable housing safely. Create a service plan to ensure ongoing services in the community.
- Work effectively with significant persons in each client’s life, i.e., family members, friends, and significant others.
- Work effectively with other EBI staff, representatives from different agencies, and community members.
- Update the program when affordable units open up, including an open waitlist for Section 8.
- Other duties and admin support as assigned within the scope of services.
