About Our Organization:
Established in 1994, East Bay Innovations (EBI) is a nonprofit human services organization that supports people with developmental disabilities, such as intellectual disabilities, autism, and cerebral palsy. Our person-centered services support people to live in their own homes, make choices about how they live, work in jobs of their choosing, and enjoy being part of their communities. About this position:
The Community Support Facilitator (CSF) oversees a caseload of individuals with developmental disabilities in our Supported Living Services (SLS) program, and is responsible for managing their daily support. The Facilitator will advocate and support our clients to make informed decisions, participate in their communities, and lead meaningful lives of their choosing. The Facilitator will hire, train, and supervise direct support staff working with our clients in their homes, and complete administrative tasks as assigned.
The Community Support Facilitator (CSF) will advocate on behalf of our clients and assist them to navigate government agencies and access public benefits. This position requires a collaborative approach to advocate for individuals with developmental disabilities, support them to be as independent as possible, and break barriers to ensure community inclusion. The Facilitator will have a strong support system within the agency, including a collaborative team of fellow Facilitators, an attentive leadership team, and ongoing training to promote growth. Qualifications:
- 5 years experience working with individuals with disabilities
- Bachelor’s degree or equivalent work experience
- Two or more years in human services, social services, or social work
- Supervisory experience
- Knowledge in Google Suite and Microsoft Office programs
Locations:
- The caseload for this position requires traveling throughout Alameda County to support our clients.
- North County - Oakland, Berkeley, Emeryville
- Central - Castro Valley, Hayward, San Leandro
- Tri-Valley - Dublin, Pleasanton, Livermore
- Possession of a valid California driver's license, a driving record that meets agency standards, auto insurance, and access to a vehicle.
Work Schedule:
The CSF will split time between supporting clients and staff in the field, and performing administrative duties in the San Leandro office.
- Full-time, 40 hours per week.
- In most cases, this will be a Monday-Friday position working during business hours.
- Overnights and weekends as needed, including holding the 24 hour Emergency Cellphone several times per month.
Client Support Responsibilities:
- Providing emotional support and companionship
- Getting exercise and walking around their neighborhood
- Connecting with friends and loved ones
- Meal planning, grocery shopping, and cooking meals
- Light cleaning and house maintenance
- Personal and intimate care (toileting and bathing)
- Wheelchair lifts and transfers
- Attend medical appointments, coordinating care, and other support as needed.
Personal Protective Equipment (PPE) is provided to all staff and clients. This includes face masks, face shields, gloves, disinfectant, hand sanitizer, and more.
Compensation:
- $76,877.00 annual salary.
- $75 stipend per 24 hours on-call when holding the Emergency Cellphone on weekdays, and $125 stipend for weekend days.

The Public Service Loan Forgiveness program is a federal program that forgives student loan debt for nonprofit employees after 120 qualifying payments.
